Notice of Privacy Practices

ABA Prep is required by law to maintain the privacy of protected health information (PHI), to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured PHI.

• Treatment. To provide, coordinate, or manage your child's care with our clinical team and, when authorized, with other providers.
• Payment. To verify benefits, obtain prior authorizations, and bill insurers or responsible parties.
• Health care operations. For quality improvement, staff training, and administrative activities that support our care.
• As required or permitted by law. Including public health reporting, mandated reporting of suspected abuse or neglect, and responses to lawful requests.

Most uses and disclosures beyond treatment, payment, and healthcare operations require your written authorization, including marketing communications, sale of PHI, and most uses of psychotherapy notes. You may revoke an authorization in writing at any time.

• Inspect and obtain a copy of your child's PHI.
• Request an amendment to your child's PHI.
• Request an accounting of certain disclosures.
• Request restrictions on certain uses or disclosures.
• Request confidential communications by alternative means.
• Receive a paper copy of this notice on request.
• Be notified in the event of a breach of unsecured PHI.

Our website contact form and intake form are not intended for the transmission of PHI such as diagnoses, treatment records, insurance card images, or other sensitive medical information. After you reach out, our intake team will provide secure channels for any information we need.

If you believe your privacy rights have been violated, you may file a complaint with our Privacy Officer at Admin@abaprep.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

We may change the terms of this notice at any time. The revised notice will apply to all PHI we maintain and will be posted on our website with an updated effective date.
